By agreeing to this registration form I, the participant/parent or guardian of registrant, agree that I will abide by the rules of the Downingtown Area Recreation Consortium (DARC). Recognizing the responsibility of physical injury associated with the activity and in consideration of DARC accepting the registrant for its activity, hereby release, discharge, and/or indemnify and agree to hold harmless the DARC, Downingtown Area School District, Downingtown Borough, East Brandywine Township, East Caln Township, Upper Uwchlan Township, Uwchlan Township, West Bradford Township, and West Pikeland Township against any claims by or on behalf of the registrant as a result of the registrant’s participation in the program. I acknowledge that DARC does not carry any accident and health insurance and assures DARC that the registrant is fully protected by medical insurance. I confirm that I have read and understand all of the policies mentioned and agree to comply with all of DARC’s policies.
